Data

To assess improvement, we will collect HEDIS Combo 2 data for your clinic. We will share this data with you at project kick-off and quarterly for a year, in aggregate and by vaccine. We will also ask you to collect data on missed opportunities every other week and data on intervention implementation every month. We will provide you with simple tools to collect this data. We will use this data to help assess the impact of your intervention and modify it, if needed.

h

Optional Future Engagement

K-CHIP will support clinics beyond the 9-month cohort with quarterly IIS HEDIS Combo 2 reports, faculty support, and QI coaching for an additional year. Clinics that opt into this program will be able to independently extend their QI project, implement additional QI projects, and monitor their outcomes.
